Care plans are individualised records that are created with the service user to provide information and preferences about their needs, actions and support, and contain any contingency plans if there is any decline in the service user's condition.Assisting the service user in maintaining as much independence as possible, care plans are in place to make sure their needs are being met to support their health and wellbeing.Social care workers should develop care plans that are easy to understand and follow, and have specific topics that should be taken to consideration to achieve a full record of the service user's situation
